Mandatory Documents

• Passport Copy: A clear copy of the passport bio page.

• Photograph: A digital photograph taken against a white background, size 35 mm x 45 mm, featuring a 70% to 80% close-up of your face.

Additional Information Required

• Medical Insurance: State whether you have a medical insurance policy valid in Russia. If yes, please provide:

  • Name of your insurance company

  • Number on your insurance certificate

• Trip Funding & Sponsorship: State how the cost of the trip is being covered. If sponsored, please provide:

  • Name of the Sponsor or Company

  • Date of Birth (if an individual)

  • Address

  • Telephone Number

• Travel History: A list of the names of countries visited in the last 3 years.

• Marital Status: Specify your status (Single, Married, Divorced, Separated, Widowed, Civil Partnership, or Dissolved Civil Partnership).

  • If Married, Divorced, Widowed, in a Civil Partnership, or Dissolved Civil Partnership, provide your spouse's/partner's details: Name, Date of Birth, Sex, Place of Birth, and Nationality.

• Parental Information:

  • Father (if alive): Name, Date of Birth, Place of Birth, and Country of Birth.

  • Mother (if alive): Name, Date of Birth, Place of Birth, and Country of Birth.

• Employment Status: Specify your current status and provide the corresponding details:

  • If Employed: Name of the Employer/Organization, Job Title, Organization Address, Email, and Contact Number.

  • If Self-Employed: Name of the Organization, Designation, Organization Address, and Contact Number.

  • If a Student: Name of the Institution, Institution Address, and Contact Number.

• Educational Details: Name and address of your educational institution.
