Philippines 9(a) Tourist Visa Document Checklist

1. Identity & Application Documents

• Valid Passport: Must be valid for at least six (6) months beyond the intended departure date from the Philippines, with a minimum of two (2) blank pages.

• Visa Application Form: Fully completed, printed, and signed.

• Passport-Size Photo: One or two recent, colored photographs (35mm x 45mm) against a plain white background, taken within the last 6 months.

2. Travel & Accommodation Arrangements

• Travel Itinerary: Confirmed round-trip or onward flight tickets/reservations.

• Proof of Accommodation: Confirmed hotel booking/reservation or an official invitation/guarantee letter from a host residing in the Philippines.

3. Proof of Financial Capacity

• Bank Records: Bank statements or certified passbooks reflecting transactions and holding sufficient funds for the last six (6) months.

4. Proof of Employment or Business Status

• For Employed Applicants:

  • Certificate of Employment (COE)

  • Latest Income Tax Return (ITR)

  • Recent salary slips

• For Self-Employed Applicants:

  • Official business registration certificates (e.g., DTI, SEC, or equivalent local business permits)

  • Latest company/personal Income Tax Return (ITR)

5. Additional Requirements for Minors (Under 18)

• Parental Consent: A notarized Affidavit of Support and Consent to Travel executed by parents or legal guardians.
